If this form is not completed within this deadline, you can not travel.

We also remind you that to travel to Italy you must:

- Present the PCR test result 72 hours before departure or the last 48 hours antigen test, this also applies to vaccinated passengers.

One of the following criteria is required for all passengers traveling to Greece:

1. COVID test, or

2. A testimony that the person has passed or

3. vaccination certificate
